There are three ways to request consulting forms:

• You can download the form in Microsoft Word format (XP version) below. After you fill out the form, save it, exit Word and then you can email it back to scl@math.uc.edu or you can mail it to our consulting lab.

• If you have difficulty with the MS-Word format, the PDF format is also available. Adobe acrobat reader is free to download for opening the PDF file. Print off the form and mail it to our consulting laboratory.

The PDF File (25K)

• You can call us at working hours (please check our weekly schedule) or just stop by the consulting lab at 512 Old Chemistry. We will talk about the project together.

A consultant or consulting team with expertise matching the need of your project will then be chosen and assigned to you, and will contact you within 2 working days to arrange an initial meeting.
